For Aged‑Care Providers & GPs
Referrals for physiotherapy under the Support at Home Program, CHSP, or a Chronic Disease Management Plan can be submitted using the dedicated referral form above and uploading the necessary supporting documuntation.
High Health works closely with your organisation to ensure your client receives the appropriate support, treatment, and documentation required for their plan.
