Funding Options

Clear, simple pathways to access the physiotherapy support you need.

High Health provides physiotherapy through several funding streams, including the Support at Home Program, the Commonwealth Home Support Programme (CHSP), Chronic Disease Management Plans (CDM), and private in‑home consultations. No matter how your services are funded, your care is coordinated with your aged‑care provider, care coordinator, or GP to ensure your treatment aligns with your goals.

Support at Home (SAH) Program

The Support at Home Program (previously the Home Care Package system) funds physiotherapy that focuses on wellness, reablement, independence, and functional goals.

How SAH Works

  • Your aged care provider sends a referral or service request

  • Goals, timeframes, and a recommended treatment plan are agreed upon

  • Sessions are delivered in your home

  • Progress is reviewed regularly to support budgeting and reablement outcomes

What SAH Can Cover

  • In‑home physiotherapy

  • Hydrotherapy

  • Equipment assessment & prescription

  • Falls prevention

  • Strength and mobility programs

Your physiotherapy is coordinated directly with your SAH provider to ensure care is compliant, goal‑driven, and fully supported.

Commonwealth Home Support Programme (CHSP)

CHSP provides entry‑level support for older adults who need help staying safe, active, and independent at home.

How CHSP Works

  • Your CHSP provider sends a referral

  • Clear goals are set based on your needs

  • Physiotherapy is delivered in your home or at the pool (if hydrotherapy is appropriate)

  • Your provider manages the funding and approvals

What CHSP Can Cover

  • Physiotherapy sessions

  • Strength and balance programs

  • Falls prevention

  • Equipment assessment, trials, and GEAT applications through the GEAT2Go Portal

Your physiotherapy is coordinated with your CHSP provider to ensure your treatment supports independence and safety.

Chronic Disease Management Plan (CDM / EPC)

Medicare‑subsidised physiotherapy with a small gap.

If you have a chronic condition, your GP may create a Chronic Disease Management Plan, which provides Medicare rebates for physiotherapy.

How CDM Works

  • Your GP prepares a CDM plan

  • You claim back a Medicare rebate for each physiotherapy session

  • A small gap applies

  • Sessions can be delivered in your home or at the pool

Common Conditions Covered

  • Arthritis

  • Chronic pain

  • Diabetes

  • Heart and lung conditions

  • Neurological conditions

  • Long‑term mobility issues

CDM can be used alongside private funding or aged‑care programs when appropriate.

Private In‑Home Physiotherapy

Physiotherapy can also be booked privately without a referral.

Ideal For

  • Clients wanting immediate access

  • People not receiving aged‑care funding

  • Those wanting additional sessions beyond funded hours

  • Clinical Pilates or hydrotherapy programs

Private sessions offer flexibility and can be tailored entirely to your goals.

  • Your support team

This ensures your care is:

  • Goal‑aligned

  • Compliant

  • Well‑documented
