High Health provides physiotherapy through several funding streams, including the Support at Home Program, the Commonwealth Home Support Programme (CHSP), Chronic Disease Management Plans (CDM), and private in‑home consultations. No matter how your services are funded, your care is coordinated with your aged‑care provider, care coordinator, or GP to ensure your treatment aligns with your goals.
Support at Home (SAH) Program
The Support at Home Program (previously the Home Care Package system) funds physiotherapy that focuses on wellness, reablement, independence, and functional goals.
How SAH Works
Your aged care provider sends a referral or service request
Goals, timeframes, and a recommended treatment plan are agreed upon
Sessions are delivered in your home
Progress is reviewed regularly to support budgeting and reablement outcomes

Commonwealth Home Support Programme (CHSP)

Your CHSP provider sends a referral

Physiotherapy is delivered in your home or at the pool (if hydrotherapy is appropriate)

Medicare‑subsidised physiotherapy with a small gap.
If you have a chronic condition, your GP may create a Chronic Disease Management Plan, which provides Medicare rebates for physiotherapy.
